Declaration of Snow Sports Participant Proficiency
To be signed by parent/legal of underage student (under 18) who elect to allow their child to participate in snow sports.
I, (name of parent/guardian) _____________________________________________ acknowledge and affirm that my child (name of child)____________________________________________________ whose date of birth is______________ has skied or snowboarded (circle one or both, as appropriate) in the past and possesses the necessary athletic skill to participate in snow sports without the necessity of professional instruction and possesses a proficiency level that allows my child to negotiate at least “green” (beginner to low intermediate) snow piste fields (aka “runs”). Further, I warrant that I have advised my child not to attempt to negotiate and runs beyond their abilities and to exercise sound judgment and that I have advised my child and my child has acknowledged he or she will obey the “Skiers/Snowboarders Responsibility Code”.
Skiers/Snowboarders Responsibility Code
1. Always stay in control and be able to stop or avoid other people or objects
2. People ahead of you have the right of way. It is your responsibility to avoid them.
3. You must not stop where you obstruct a trail,or are not visible from above.
4. Whenever starting downhill or merging into a trail, look uphill and yield to others.
5. Always use devices to help prevent runaway equipment.
6. Observe all posted signs and warnings. Keep off closed trails and out of closed areas.
7. Prior to using any lift, you must have the knowledge and ability to load, ride and unload safely.
